We provide in-center hemodialysis treatments utilizing NxStage home hemodialysis equipment that enables our patients to experience the difference of shorter frequent dialysis. Our goal is to empower patients and their physicians with the option to chose a treatment that promotes a more flexible lifestyle and improved outcome. Dialyze Direct trained clinicians administer the therapy using the NxStage System One. During this time, the patient will have the opportunity to collaborate with the Dialyze Direct care team to determine whether a transition from in-center to home hemodialysis is a suitable option.
Additionally, we offer in-center short term respite care for home hemodialysis patients that want to give their care partner a break.
A transition to Dialyze Direct’s Home Hemodialysis Training and Support Program involves you and a care partner (spouse, family member or friend). Our Care Team will provide the necessary training and education to facilitate the transition to home hemodialysis, permitting you to enjoy the benefits of dialysis in the privacy and comfort of your home. During the training period, you and a care partner will build the necessary skills and confidence to begin performing home hemodialysis, enabling you to regain control of your life. The Dialyze Direct Care Team will provide continuous training and support in order for you to feel comfortable dialyzing at home. At Dialyze Direct, we believe patients should be able to live with dialysis rather than for it.
Dialyze Direct provides staff-assisted home hemodialysis within the long-term care and rehabilitation facilities. Using the most advanced NxStage equipment, treatments are tailored to help patients make the most of their time spent in the skilled nursing and rehabilitation facilities. Patients benefit from our ComfortCare™ Model provided by the Dialyze Direct Care Team. Under Dialyze Direct care, we perform shorter frequent treatments helping the patient avoid the burden of tiresome travel to the outpatient center. Our interdisciplinary team collaborates with the skilled nursing/rehab facility care team and your nephrologist to coordinate a plan of care to improve patient outcomes.
Shorter frequent hemodialysis is a different way of doing hemodialysis. Instead of having to go to a dialysis center 3 times a week for treatments that are about 4-5 hours long, plus recovery time that can take up to 8-9 hours; a person doing home hemodialysis does his or her treatments at home. There are different types and schedules of home hemodialysis, but the most common is more frequent home hemodialysis, in which the patient does shorter treatments (about 2.5 hours) more often (5 times per week) at home. The typical recovery time for this type of treatment is about 1/2 hour.
Frequent dialysis keeps the amount of toxins and water build-up to a minimum, which makes the treatment shorter and may reduce or eliminate many side effects dialysis patients typically experience. The patient and a care partner (usually a family member or friend) are trained by the dialysis center nurses to do their treatments safely at home. Doing dialysis more often is closer to how healthy kidneys work. Many patients report – and various studies have confirmed – that compared to three-times-weekly in-center hemodialysis, frequent home hemodialysis may offer the following health and quality of life benefits:

When a patient is discharged, coordination of care is almost as important as continuity of treatments. That’s why our Interdisciplinary team will work with the long term care staff/ rehabilitation center to ensure continuation of care that will focus on better outcomes and reduce re-hospitalizations.
Collaboration: A Key To Success
Dialyze Direct’s model of care uses a frequent HD modality that provides many clinical benefits: post dialysis fatigue is significantly lessoned, volume status comes much closer to ideal and hypertensive and phosphate binding medications are typically decreased. There is significant data to suggest that hospitalizations and re-hospitalizations are reduced. These benefits span across all age demographics and is especially critical to the geriatric population.
This is important because the frail and elderly often return to the nursing home from a hospitalization in a diminished physical condition. We utilize NxStage technology and patients average five (5) treatments a week with treatment times averaging ~2 hrs 45 minutes. With respect to patients residing in the nursing home, having hemodialysis on site will help patients avoid missed meals, missed physical therapy sessions and will reduce the considerable physical toll of travel and transportation issues that these patients experience. In addition to delivering an excellent dialysis treatment, by being on site almost every day, we make it a priority for our staff to coordinate care and cooperate with the nursing home staff; something that has been vary hard to achieve at arms-length distance when patients travel to a remote dialysis.
